You may download and print these forms prior to your first visit. We know your time is valuable. The forms contain all information necessary for our staff to assist you and save you time on your appointment day. Any outside medical documentation from other specialists may be brought the day of the appointment, or faxed directly to our office ahead of time. (Fax: 231-947-0977) Some questions on the forms may be hard to answer, but please try your best to answer them all.

Traverse City Office
Our office is a brown brick building located next to the Traverse City Public Library overlooking Boardman Lake. We are the front office building closest to Woodmere Avenue. Several handicapped parking spots are available directly in front of the walkway leading up to the front office door which faces Woodmere Avenue.
Our Manistee office is located on the Manistee River next to House of Flavors and attached to (and behind) the Edward Jones Office. Parking and access to the office is located in the back of the building.
Please arrive 20 minutes prior to your scheduled appointment time if you have not already completed your New Patient Forms. If you have already taken the steps to fill these forms out ahead of time, please arrive 10 minutes prior to your appointment time.
If you will not be able to make your scheduled appointment time, please call the office to notify us by calling 231-946-8822 (Traverse City Office) or 231-723-7407 (Manistee Office).
- When scheduling your appointment, it’s extremely helpful if you let our stuff know anything and everything you’ll be coming in to see us for ahead of time. Some patients have several issues, and we want to make sure we set aside enough time to address all your concerns. Appointment duration is catered to you, the patient, not a premade schedule on a computer!
- Wear comfortable clothes if possible. We understand some of our patients’ appointments are made during their work day, so we completely understand if you need to arrive in work attire.
- Bring a few pairs of shoes! Part of understanding your issue might involve looking at what kinds of shoes you wear on a daily basis. Our shoes tell us a story about how we walk. Assessing for extra wear and tear in certain areas of your shoes helps us understand how you walk, why you might be in pain, and what we need to do to get you feeling better quickly!
